PE Intervention Log

The PE Intervention Log is a printable instrument for tracking each operating-partner intervention against the cluster it was meant to fix and its Hundred-Day Echo. Use one row per intervention, in the order they were made, in the words the team would recognise.

The point of the log is not to grade the operating partner. The point is to make visible — to the operating partner, to the portco CEO, and to the investment committee — which decisions have actually changed structure and which have only changed slides.

How to use it

  1. List every operating-partner intervention from the last two quarters, in chronological order.
  2. For each one, name the cluster the intervention was meant to fix and the architecture it belonged to.
  3. Record the Three Doors decision (Remove, Defer With Clarity, or Accept) and the date the team was told.
  4. After 30, 60, and 100 days, mark whether the cluster has continued to echo — fresh frustration submissions matching the same pattern.
  5. Bring the log to the next operating review. The decisions with high Echo rates are the decisions that have not landed for the team.
