The Confidential CEO Survey is a structured instrument for the operating-partner / portco CEO conversation. It is not an engagement instrument and it is not benchmarked. It is twelve open-text prompts the CEO answers in their own words, in writing, before the operating review.
The point of the survey is to give the CEO a structured place to name the frustrations they themselves are inside — bottlenecks, loops, churn, ambiguity, constraints — without having to lead with them in a meeting. The operating partner reads the responses before the conversation, not during it.
How to use it
- The CEO completes the survey in writing, alone, before the operating review. Allow 45-60 minutes.
- Responses go to the operating partner. They are not shared with the board, the management team, or the portfolio company in raw form.
- The operating partner reads the responses before the meeting. The meeting opens with the cluster the CEO has named, in the CEO's words.
- Repeat once a quarter. Track the architectures that recur — the recurring architecture is the structural read.
The instrument — twelve prompts
1. What recurring decision in this business consistently fails to get made, or gets made too late? Who is the named owner of that decision?

2. What approval gate currently exists in this business that you yourself believe is no longer necessary, but you have not yet removed?

3. What was the top operating priority of this business six months ago, and what did the team actually finish before it changed?

4. Name an outcome two of your direct reports each believe they own. What has it cost the team for that ambiguity to remain?

5. Name a constraint that everyone in your operating team knows is real and that nobody is willing to write down in a meeting.

6. Of the operating-partner interventions in the last 100 days, which one has not yet landed for the team? How do you know?

7. Which decision in the last quarter did you walk through the room in Three Doors voice — "I can fix that. Here's what I'm going to do" — and what changed for the team?
8. Which decision in the last quarter would the team say you announced but did not actually structurally change?
9. Which member of your team has stopped raising structural issues that they used to raise? When did that change?
10. What is the one structural change you have authority to make this quarter that you have not yet made? What is holding you back?
11. Where in the value-creation plan are you most confident the lagging engagement score will eventually catch up to a leading frustration signal you can already see?
12. What would you want the operating partner to read here that you would not want to say out loud in the operating review?
